In order to apply for the pharmacy summer camp, first a log-in must be created using your email address and social security number. It is important that the email address chosen is active and checked often, as camp correspondence will be through email.
There are two parts to a complete camp application: The online application and a grade verification form. BOTH must be completed and submitted on time in order to be considered for admission to the camp.
Your information is kept confidential which requires you to login. Please type in your SSN and password to begin. If this is your first time, fill in your SSN and select a password you will remember. You will be prompted to confirm your password and you must specify your email address. You can navigate to other parts of the web site from here and return later to login.
New logins will not be accepted after April 14, 2008 at 5 p.m., however once setup, you may login anytime afterwards for information and printing of your application.
