Requirements:
The Ohio State Board of Pharmacy requires that a student be actively working towards the requirements for licensure as a pharmacist in order to apply for an intern license. The State Board defines ‘actively working' as having begun the PharmD classes. For the PharmD program in the College of Pharmacy at The University of Toledo this means the student has to have started attending third year classes in the Fall Semester.
This means that a student accepted into the PharmD program in April who is to begin PharmD classes in August will not be eligible for licensure until the student has begun classes in August. If you have questions about this process please contact the Ohio State Board of Pharmacy at 614.466.4143.

Enrollment Verification:
You will need to obtain an enrollment verification form from the Office of Student Affairs (WO 1227). Enrollment verification forms will be prepared for all new third year students who will begin their professional division coursework in the Fall semester. They may pick up their completed form beginning the first week of Fall semester. Other students enrolled in the third year or higher needing an enrollment verification form for an intern license application may request the form from the Office of Student Affairs (WO 1227).
